Space shuttle Challenger exploded 73 seconds after liftoff from Cape Canaveral, killing all seven crew members, including school teacher Christa McAuliffe.
A faulty O-ring seal sparked the breach that led to the explosion. The shuttle fleet was grounded for nearly three years, during which various safety measures, solid rocket booster redesign and a new policy on management decision-making for future launches were implemented.
